Please fill the necessary forms completely as they are needed to provide us with as much information as possible to determine your root cause issues. Keep in mind, this is a no judgement zone. You will be treated with the utmost care and concern.  In our discussions, you may be reminded of things you hadn't remembered and we can always add that detail to your specific file.  **Your information is completely confidential and will not be shared with anyone else.**

Most of these forms can be filled out electronically and emailed back to me.  A few may need to be printed and brought to your next session, or scanned and emailed.  Some will be filled out in our sessions together.  You are only responsible for providing what may be useful in your situation only so not all forms may be necessary.  The only form I need at the start is the completed "Client Intake Form".  I will need that before we can schedule our first session together!
